Glass Activity Waiver and Release of Liability

Effective Date: June 2025

 

1. Acknowledgment of Risk

I understand that participation in any glass-related activity at Art on Fire Studio — including but not limited to cutting, scoring, breaking, handling, grinding, and assembling glass — involves inherent risks of injury. These risks may include, but are not limited to:

  • Cuts, punctures, or abrasions from sharp glass edges;

  • Exposure to airborne glass dust or small shards;

  • Burns or heat-related injuries associated with the kiln;

  • Slips or falls from debris or spills;

  • Damage to clothing or personal property.

I acknowledge that even when safety protocols are followed, injuries may still occur.

 

2. Assumption of Responsibility

I voluntarily assume full responsibility for my own safety (and/or for the safety of the minor named below) while participating in glass-related activities at Art on Fire Studio. I agree to follow all safety rules, wear protective eyewear when instructed, and use tools and materials only as directed by staff.

I further agree to immediately report any injury, unsafe condition, or broken glass to an instructor or staff member.

 

3. Release and Waiver of Liability

In consideration of being permitted to participate in glass activities at Art on Fire Studio, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and hold harmless Art on Fire Studio, its owners, employees, and agents from any and all claims, demands, damages, or causes of action arising out of participation in glass-related activities — including those resulting from ordinary negligence.

This release includes, but is not limited to, injuries sustained during participation, handling of glass, use of tools or equipment, or presence on the premises.

 

4. Participant Obligation and Conduct

I agree to:

  • Use all materials and equipment responsibly;

  • Follow the instructions of studio staff at all times;

  • Refrain from horseplay, disruptive conduct, or unsafe behavior;

  • Maintain awareness of surroundings and others working nearby;

  • Not touch or handle another participant’s materials or artwork.

Failure to comply with safety guidelines may result in removal from the activity without refund.

 

5. Parental or Guardian Responsibility

If I am signing on behalf of a minor participant, I certify that I am the child’s parent or legal guardian and have the authority to sign this release.
I acknowledge and accept full responsibility for the minor’s conduct, safety, and adherence to all studio rules. I understand that Art on Fire Studio staff are facilitators, not child supervisors, and that I remain fully liable for the actions of the child in my care.

 

6. Artwork and Kiln Disclaimer

I understand that glass art is inherently unpredictable during the kiln-firing process. Variations in color, texture, and shape — as well as cracking, bubbling, or breakage — can occur naturally. Art on Fire Studio cannot guarantee specific outcomes or replace damaged work resulting from the firing process or handling.

 

7. Photography and Media Release (Optional)

I grant permission for Art on Fire Studio to photograph or record participants and artwork for marketing and promotional use, including on social media, websites, and print materials.
